Immunizations

Per Indiana Code 20-34-4, Updated Immunization records (or a letter expressing religious or medical exemption) must be up-to-date for a student to attend school in Indiana.  Please be sure to give a copy of all your children's updated immunizations to your school nurse or to the MSDLT Welcome Center.

3. Walgreen's Pharmacy - Will bill private insurance and Medicaid for students 11 years and older.

4. Do you need a medical or religious waiver form for immunization status? These need to be filled out yearly and turned into your school.  Remember your student could be excluded from school is there is an outbreak of a vaccine preventable disease.  If you have any questions, please contact your school nurse.
